Do not include personally identifiable information (PII) in this message. PII is data that, when provided alone or in combination with other data, indisputably identifies an individual person. Examples of PII:

• Teacher name and teacher ID number
• Student name and student ID number
• Name and date of birth
• Name and home address
• Social security number (SSN)
• Screen shot showing any of the above
